Discovery Health is a non-profit organisation and registered with the Council for Medical Schemes. You’ll find a Discovery Health application forms on their website. You’ll find every kind of application form to join at Discovery and from other sources too. It’s hugely beneficial as you’ll find application forms to –

• Change bank details
• join Discovery Health as part of an employer group
• join Vitality
• register a newborn baby and many other application forms

These forms also contain some rules for becoming a member with them. It is up to you to read these rules and also to understand them, as there are exclusions and waiting periods that need to be understood.

• Fill in the form and print clearly
• You’ll notice questions such as –

•  Provide details about yourself, your spouse and your dependents
• details about your financial adviser
• what plan you like
• your employment
• your banking details

 

• Sign the sections they tell you to. Just as an example, if you are applying to become a KeyCare member, you’ll have to sign section 6, 8, 12 and 13
• Email your completed- and signed form to application@discovery.co.za. You can choose to fax it to 011 539 3000
• you must attach a copy of each applicant’s identity document
• once your application form has been sent it, your membership will be activated, and you will receive an acceptance- and welcome letter
• once you send Discovery your application form, they will let you know if certain details are missing or even if they require additional information for underwriting purposes
